Insurance and Payment Information

The following are accepted for payment: Cash, Check, Debit card, Credit card, HSA cards, CareCredit.

Most major insurance plans are accepted; terms apply as per your individual policy as set by your insurer which includes co-payments and deductibles. Please provide this information BEFORE the date of your scheduled appointment to allow us ample time to verify your benefits and coverage. Types of Office Visits

Please be advised that office visits are categorized by CPT codes (Current Procedural Terminology Codes). These codes provide a uniform way across the nation for coding medical procedures and medical services. At Oak Hills Women’s Center, P.A., we comply with this coding system for administrative purposes which includes the classification of visits and the filing of claims for services. For example, annual wellness visits have their own specific CPT codes. Problem visits also have their own specific CPT codes. Services provided at an annual wellness visit are obviously not the same as those services provided at the time of a problem focused visit. Therefore, a routine annual wellness exam/visit is not a problem focused exam/visit and are not combined since they are coded differently. If you wish to have a routine annual wellness examination/services, then please schedule for an annual wellness exam. If you have specific concerns or problems, we recommend that you schedule a problem visit.  We appreciate your understanding and cooperation to allow us to better serve you.

Scheduling an Appointment

To schedule an appointment, please call our office telephone number at (210) 692-3636 during routine office hours. Our staff will be happy to assist you. When you schedule your appointment be prepared to provide the following information:

  1. Your name and date of birth
  2. The reason for your visit
  3. Details regarding your currently active medical insurance coverage including the exact name on the policy and the policy number for verification.

On the date of your appointment, if you are a new patient please arrive at least 30 minutes early and bring with you the following:

  1. Photo identification, such as a driver’s license or other legal photo ID
  2. Your current insurance card
  3. Please accurately and completely answer the medical forms below and provide them to be placed into your medical records in order to expedite your visit.
  4. Provide a list of all medications, names and dosages that you take
  5. List of any allergies that you may have.
  6. List of all surgeries and hospitalizations.

Please remember that your cooperation in this matter allows us to better serve you so that you may have a more positive experience.

Please be advised that children under the age of 12 years are not allowed in the office.
